The practice are now inviting all patients who either have a long term condition or who take medications that require monitoring to attend for an annual review. The review is to give you an opportunity to discuss your condition. It is also a chance to check that you are on the best treatment and to monitor your medications.
The review will include some questions and some measurements. Exactly what we do depends on your condition, but it might include checking your blood pressure or taking a blood test.
We are arranging your review date to the month of your birth to make the process easier to remember. This may mean you need to attend a little earlier than you have in the past.
